What is this site for?
The Website www.forms.gov.bd is the one of the e-initiatives of the Government of Bangladesh. As a part of the e-Citizen service initiative, the Prime Minister's Office has undertaken to digitally publish the government forms to make them available to citizens through this website. This website provides you with government forms that can be selected, viewed, downloaded or fill electronically and print or just print a blank form and fill it manually for the submission to the respective office/agency. In this pilot phase we put 50 forms and eventually the rest of the forms will be available in future.

Can I file  the forms downloaded from this website?
Yes, all the Government forms available for download on this site are approved for use.

How can I locate a particular a form?
You can easily and quickly reach to the required form by navigating forms in more than one way. You can select to move with respect to Category, Ministry/Divisions, or you can also select to view all forms altogether in alphabetical order. You are also allowed to search for certain specific forms.

When I download the form it does not open?
In this site, forms can be downloaded in two formats. The PDF format and the other is doc format. If you try to download form in the PDF format then you must have Acrobat Reader installed on your system. Similarly if you choose to download forms in doc format then you must also have some word processing software such as MS Word, Open Office, Star Office etc to open it.

After downloading a form, how can I open the file to read it?
To view/print a PDF file from the Web, download the file to your hard drive or diskette, launch the Acrobat Reader (an icon will be created during the setup process) and open the file you just saved.  You can download the Acrobat Reader from
http://www.adobe.com. It is a freeware. To read the file in doc format you need a word processor which can open doc format. Your computer must have MS Word/Open Office installed.

All of our files are verified prior to posting on the web site. Most likely your download was not successful. Try to download the file again. However, if you still face the same problem, please inform us.

Forms are in different formats in this site. Which format I need?
Its depends on how you want to fill the form. If you want to fill the forms electronically, then you need the fillable form. These are available in PDF (English) and in doc (Bangla and English) format.

If you want to fill the forms manually you can download the blank form available in both PDF and doc format.

Forms are available in different paper sizes, do I need different size paper to print different forms?
No you dont. All the forms available in this website are converted into A4 size. You need only A4 size papers to print the forms.

There is no restriction. You can print the form on one side or on both side of a paper.

Generally all the government agencies will accept forms that are consistent with the official forms and that do not have any adverse impact on processing your form. This policy includes forms printed on high quality devices such as a laser, bubble-jet or ink-jet printer etc. Forms must be printed using good quality paper, ink, and with precise specifications so that forms are highly legible.
