As a part of the e-Citizen services initiative of the Government of Bangladesh, the Prime Minister's Office has undertaken to digitally publish the government forms to make them available to citizens through this website and CD-ROM. This website provides you with government forms that can be selected, viewed, downloaded or filled electronically and printed for submission. Non-electronic forms available can be printed and manually completed for submission to respective office/agency.

In this website, forms are available in blank format and in fillable format.  If you want to fill the form manually, download the blank form and print it. Blank forms are available in both PDF and doc format.

To fill forms electronically, you need to download fillable forms. These forms are available in both PDF (English) and doc (English and Bangla) format.

To fill forms electronically in Bangla you will require Bangla software and Input system.
